Saline vs. Silicone Breast Implants

The major consideration when seeking breast augmentation surgery is the type of implant you want Dr. Barber to use. There are two fillers for breast implant devices in wide use, silicone gel and saline. Saline enjoyed pre-eminence in the United States during a fourteen-year long ban of silicone gel implants while the Food and Drug Administration (FDA) conducted extensive evaluations. Since November of 2006, both products are now available at the Pittsburgh plastic surgery clinic.

Saline

Saline implants are made out of a silicone elastomer, which can be placed while partially empty during surgery, and then filled with saline (salt water) after placement.  Because the implant is deflated, this helps reduce the size of the incision and minimizes the scarring that occurs.

Saline is more readily absorbed by the body in the event of a rupture or leak. Rupture and possible adverse affects of silicone gel implants were the main reasons silicone was banned by the FDA.

During the 1990's saline implants were used widely in the United States, but were not widely used in other countries because of the availability of silicone gel implants.

Drawbacks of Saline Implants

  • More susceptible to leaks
  • Sometimes shows a "rippling" effect
  • When ruptured, reduction in size and shape of breast is obvious

Silicone Gel

Most people seem to prefer silicone gel implants for their aesthetic value. They look and feel more natural. The latest silicone gel implants have been in use in other countries for many years, have been in constant use for breast reconstruction following mastectomies, and are now approved by the FDA and available to all women over age 22. Silicone gel devices contain highly cohesive gel; meaning that in the event of a rupture, there is little chance of the fluid actually leaking out of the implant shell.

Drawbacks of Silicone Gel

  • Potential health problems
  • Unnatural appearance (less so in later generation products)
  • Still susceptible to rupture
  • Follow-up monitoring required
